Transcript and Reference Request Form
For children in formal school programs, Caedmon requires a completed ISAAGNY form for each applicant. Please provide your child’s school with the Transcript and Reference Request Form. This will give your child’s school permission to send us reports. We require that the confidential ISAAGNY form be sent directly from your child’s current school to The Caedmon School.
Educational Records Bureau
All applicants to Kindergarten through fifth grade are required to complete testing conducted by the Educational Records Bureau (ERB). If your school does not offer this assessment please contact the ERB at 212-672-9800 to schedule an appointment.
