Welcome to the Caedmon School health services web page. Please find below a list of required forms and other information used by the health office at Caedmon.
All forms for returning students must be updated yearly. Please call the nursing office if your are unsure of the date of your child's last physical exam.
Required
Child and Adolescent Health Examination Form. This form is required from every student once annually. No student is permitted to attend school without having a current health form on file. (A Pediatrician's computer generated annual exam form will also be accepted).
Medication List. This form is required from all families prior to their child's first day of school. It may be updated and resubmitted by the family as needed.
Emergency Medical Treatment Authorization. This form is required from all families prior to their child's first day of school.Consent remains valid until revoked by parent in writing or for the duration of attendence.
Medication Administration Form. This form is only required from families who wish to have medication provided at school by the school nurse or health director.

Asthma Action Plan
Food Allergy Action Plan. If a child is diagnosed with a life-threatening food allergy, the school requires that child to have this form on file, along with any anaphylactic medication (i.e. epinephrine or Benadryl).
